Custom vs. Off-the-Shelf Knee Braces: Which Do You Need?
By Eric M. Rippetoe, PT, DPT, physical therapist and founder of BISHOP · Reviewed September 28, 2026
SHORT ANSWER
Off-the-shelf knee braces come in standard sizes and work well for many people, especially for mild or short-term problems and typically shaped legs. A custom-fabricated brace is built from measurements or a 3D scan of your leg, and it makes the most sense when a standard brace won't fit or stay in place: for example, when your thigh and calf are very different sizes, your leg or knee is misaligned, you have little muscle for the brace to grip, or ongoing instability isn't controlled by a brace you've tried.
The main types of knee braces
- Sleeves and wraps: compression and warmth with little mechanical support.
- Hinged off-the-shelf braces: side hinges and straps that limit side-to-side motion, sold by size.
- Off-the-shelf functional and unloader braces: more rigid designs for ligament instability or arthritis, sized and sometimes adjusted by a clinician.
- Custom-fabricated braces: made for one person from that person's measurements, cast, or 3D scan.
Signs an off-the-shelf brace isn't working
- It slides down your leg (migration) and has to be pulled back up during the day.
- The straps or edges pinch, rub, or leave gaps against your leg.
- Your measurements fall between sizes, or the thigh fits while the calf doesn't.
- The knee still gives way while you're wearing it.
- You stop wearing it because it's uncomfortable, which means it isn't helping.
Who tends to benefit from a custom brace
Custom fabrication is most useful when the shape of the leg is the problem. Medicare's knee brace policy gives the same kinds of examples: deformity of the leg or knee, the size of the thigh or calf, and minimal muscle mass on which to suspend a brace. People with larger or very lean legs, significant bowing or knock-knees, or a history of braces migrating are common candidates.
Custom doesn't automatically mean better. If a well-fitted standard brace controls your symptoms and you'll wear it, it may be all you need.
Tradeoffs to weigh
- Cost: custom braces from the major brace makers commonly sell for about $500 to $1,500 or more, often plus a fitting visit. BISHOP's custom brace is $299.99 per knee.
- Time: a custom brace has to be designed and built, so it isn't available the same day.
- Fit data: someone has to capture your leg's shape accurately, by measuring, casting, or scanning.
How a BISHOP brace is made
BISHOP braces are designed from a 3D scan of your leg that you capture at home with any iPhone that has Face ID, so there's no fitting appointment. The brace is modeled to follow the contours of your leg (total contact), 3D-printed, and finished by hand in Shreveport, Louisiana. It costs $299.99 per knee plus shipping, ships 1 to 4 days after payment, and comes with a 3-year warranty and a 30-day fit guarantee.
Common questions
- Is a custom knee brace more comfortable than an off-the-shelf brace?
- Often, because it's shaped to your leg, which spreads pressure evenly and helps it stay in place. Comfort still depends on design, materials, and wearing it correctly.
- Do I need a prescription for a custom knee brace?
- Not usually. Many custom braces are sold through a clinic after a fitting visit, but you can order a BISHOP brace directly without a prescription. If you don't know what's causing your knee problem, an evaluation by a physician or physical therapist is still worthwhile.
