Notice of Privacy Practices

Effective: October 27, 2026

Rippetoe Health, Inc. d/b/a BISHOP 415 Texas St., Ste. 310, Shreveport, LA 71101 · 318-333-1331

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

BISHOP makes custom orthotic braces. To do that, we create and receive health information about you, such as your name, contact details, measurements and 3D scans, your diagnosis and your prescription. The law calls this “protected health information.” This notice explains how we may use and share it, your rights, and our duties.

How we use and share your health information

For your treatment, payment and our operations

We may use and share your information without your written permission for:

  • Treatment. To design, make, fit, adjust and repair your brace, and to work with the doctors, therapists and partner offices involved in your care. Example: we share your scan and fitting notes with the physician who prescribed your brace.
  • Payment. To bill and get paid by Medicare, the VA, your insurer or you. Example: our billing service sends a claim with your diagnosis and the brace we supplied to your health plan.
  • Health care operations. To run our business and improve quality. Example: we review orders and fittings to check that braces meet our standards and to train our staff.

Service providers. Companies that help us, such as our billing service and the secure email and file storage we use, may receive your information. They must sign agreements to protect it.

Reminders and updates. We may contact you by phone, text or email about your order, fitting, delivery or follow-up care.

People involved in your care

We may share information with a family member, friend or caregiver who is involved in your care or helps pay for it, unless you tell us not to. If you can't tell us your preference, for example in an emergency, we may share it if we believe it is in your best interest.

Other uses the law allows or requires

We may also use or share your information:

  • When the law requires it.
  • For public health and safety, such as reporting problems with medical devices to the FDA, or preventing disease.
  • To report abuse, neglect or domestic violence to authorities, as the law allows or requires.
  • For health oversight, such as Medicare audits, accreditation surveys, inspections and investigations.
  • For lawsuits and legal actions, in response to a court order, subpoena or other lawful request.
  • For law enforcement, as the law allows.
  • To avert a serious threat to your health or safety or someone else's.
  • For special government functions, such as military and veterans' activities and national security.
  • For workers' compensation claims.
  • To coroners, medical examiners and funeral directors, and for organ and tissue donation.
  • For research that meets the law's privacy protections.
  • To the U.S. Department of Health and Human Services when it checks our compliance.

If Louisiana or other law gives your information more protection than federal law, we follow the stricter law.

Information we share as described in this notice may be shared again by the person who receives it and may then no longer be protected by federal privacy law.

Uses that need your written permission

We will not do the following without your written permission:

  • Marketing. Use your information to market products or services from others, or be paid to send you marketing.
  • Selling your information. We never sell your health information.
  • Psychotherapy notes. We do not keep these, but if we ever did, sharing most of them would need your permission.

Any other use or sharing not described in this notice also needs your written permission. You may cancel a permission at any time by writing to us. Cancelling stops future sharing, but not sharing we already did in reliance on your permission.

Your rights

To use any of these rights, contact our Privacy Officer (below). We may ask you to make the request in writing.

  • Get a copy of your records. You may see or get a paper or electronic copy of your health information. We will respond within 30 days and may charge a reasonable, cost-based fee.
  • Ask us to correct your records. If you think information is wrong or incomplete, ask us to correct it. We may say no, but we will tell you why in writing within 60 days.
  • Ask for confidential communications. You may ask us to contact you a certain way (for example, a different phone or email) or at a different address. We will agree to reasonable requests.
  • Ask us to limit what we use or share. You may ask us not to use or share certain information for treatment, payment or operations. We don't have to agree, except: if you pay for an item or service in full out of pocket, you may ask us not to share information about it with your health plan, and we will agree unless the law requires us to share it.
  • Get a list of those we've shared with. You may ask for a list of times we shared your information in the six years before your request, who we shared it with and why. It won't include sharing for treatment, payment or operations, or sharing you asked us to make. One list a year is free; we may charge a reasonable fee for more.
  • Get a paper copy of this notice at any time, even if you agreed to receive it by email.
  • Choose someone to act for you. A legal guardian or someone with your medical power of attorney may use your rights for you. We will check that they have that authority.

Our duties

  • The law requires us to keep your health information private and to give you this notice of our legal duties and privacy practices.
  • We will tell you promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the notice currently in effect.

Changes to this notice

We may change this notice, and the changes will apply to all information we have about you. The new notice will be on our website at bishop3do.com, available at our office, and sent to you on request.

Complaints

If you believe your privacy rights were violated, you may complain to us or to the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
